29 CFR  1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means:   a) Job site, at/near 4759 Campbells Run Road, Pittsburgh, PA: On or about July 21, 2023, during masonry activities, the employer did not ensure that a scaffold, deemed to be approximately 20 feet tall and 4 feet wide, was appropriately restrained from tip over hazards.

29 CFR  1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms:    a) Job site, at/near 4759 Campbells Run Road, Pittsburgh, PA: On or about July 21, 2023, during masonry activities, the employer did not ensure that appropriate fall protection systems/devices were in use where employees were exposed to fall hazards of up to approximately 13 feet in that some areas of the scaffolding had no guardrail systems in use and other areas only had a toprail in use.
