Location - Roof of a three story building. On or about 1/24/2023, employees were not instructed to wear the PFAS correctly while working at heights greater than 6 feet. The lifeline was attached to the wearer's side/front dee ring to keep the lifeline in front of the employees.

The Randy AP Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1635188, citation number 1, item number 1c and was affirmed as a final order on 6/19/2023, with respect to a workplace located at 7 Taunton Street Plainville, MA 02762.
