29 CFR 1910.1030(d)(4)(iii)(A)(4): Reusable containers for contaminated sharps were opened, emptied, or cleaned manually or in another manner which exposed employees to the risk of percutaneous injury:  On or about July 24, 2023, employees used and stored sharp container systems that had to be manually opened while performing tasks that included, but were not limited to, catheter insertions and injection administration. 29 CFR 1910.1200(e)(1)(i): The employer did not compile a list of the hazardous chemicals known to be present using a product identifier that was referenced on the appropriate safety data sheet.  Facility-wide:  On or about July 24, 2023, the employer failed to compile a list of hazardous chemicals known to be present using a product identifier that references the appropriate safety data sheet for the products used. Products regularly used by employees included, but was not limited to, a grooming blade cleaner called Oster Blade Wash and a hazardous liquid drug used for local anesthesia, Bupivacaine Sulfate.

29 CFR 1910.1200(h)(3)(iii): Employee training did not include the measures employees can take to protect themselves from chemical hazards, including specific procedures the employer had implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures and personal protective equipment to be used:   On or about July 24, 2023, training provided by the employer did not include protective measures that employees could take that were specific to the procedures performed while handling harmful chemicals.
