On or about July 24, 2023, at the above addressed jobsite, an employee using a pneumatic nail gun was not wearing safety glasses, thereby exposing the employee to hazards associated with eye injuries.

On or about July 24, 2023, at the above addressed jobsite, employees performing residential roofing operations, nineteen (19') feet above the ground, were not protected from falls, thereby exposing employees to fall hazards.
