OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees to employees exposed to struck-by hazards from the use of a lifting hook with damages or missing safety latch: a) At the jobsite located at 7049 Greenbrier Parkway NW, Madison, AL: On or about 07/25/2023, employer exposed employees to struck by hazards in that the Gorbel jib crane in Zeus crew line with a lifting device (hook) that had damaged safety latches. b) At the jobsite located at 7049 Greenbrier Parkway NW, Madison, AL: On or about 07/25/2023, employer exposed employees to struck by hazards in that the Gorbel jib crane in Zeus 3-seat line with a lifting device (hook) that had damaged safety latches and were using zip ties to hold the hook onto lifting device.

Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked. a.) At the jobsite located at 7049 Greenbrier Parkway NW, Madison, AL: On or about 07/25/2023, employer exposed employees to struck by hazards in that aisles were not kept clear and unobstructed near welding production.
