29 CFR 1926.451(g)(4)(iv):When midrails are used, they shall be installed at a height approximately midway between the top edge of the guardrail system and the platform surface. On or about July 14, 2023, at a worksite located at 1300 Maria Drive, Stevens Point, Wisconsin: Employees were exposed to a fall hazard of approximately 13 feet from a missing mid rail on scaffolding.

29 CFR 1926.454(a):The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable: On or about July 14, 2023, at a worksite located at 1300 Maria Drive, Stevens Point, Wisconsin: the employer did not develop and implement a scaffold training program to enable employees exposed to fall hazards during construction activities to recognize the hazards of falling and procedures to be followed in order to minimize those hazards.
