29 CFR 1926.403(b)(2): Installation and use. Listed, labeled, or certified equipment shall be installed and used in accordance with instructions included in the listing, labeling, or certification. On or about July 14, 2023, at a worksite located at 1300 Maria Drive, Stevens Point, Wisconsin: Employer did not ensure flexible extension cords were used in accordance with the manufacturer's instructions. Two flexible extension cords were plugged together (daisy chained) to power a hand-held drill.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables shall be protected from damage. Sharp corners and projections shall be avoided. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage. On or about July 14, 2023, at a worksite located at 1300 Maria Drive, Stevens Point, Wisconsin: employer did not ensure the extension cord being used to power a hand held drill that was extended across the lot was protected from damage where the JCB loadall telehandler (Model: 510.56 F, Product Identification Number: JCB5CG2GJN3178763) was parked on top of the extension cord.
