29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   859 Stoney Hill Rd., Wilbraham, MA: On or about July 25, 2023, several employees were observed working on a steep slope roof engaged in roofing activities with unprotected sides and edges greater than 10 feet to the next lower level, asphalt ground, and were not protected by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  Juan Garcia was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1616902, Citation number 2, Item number 1, and was affirmed as a final order on 12/30/2022, with respect to a workplace located at 195 Russell Street, Hadley, MA.    Juan Garcia was previously cited for a violation of an equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1390366, Citation number 1, Item number 1 and was affirmed as a final order on 6/19/2019, with respect to a workplace located at 158-162 Church St., Chicopee, MA.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards:  859 Stoney Hill Rd., Wilbraham, MA: On or about July 25, 2023, employees were exposed to fall hazards are part of their assigned duties and the employer had not trained them prior to exposure to these hazards in topics such as but not limited to recognition of fall hazards, dangers of fall hazards, and procedures each employee must take to prevent or minimize risk of such exposures.  Juan Garcia  was previously cited for a violation of this occupational safety standard, 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1616902, Citation number 1, Item number 2 and was affirmed as a final order on 12/30/2022, with respect to a workplace located at 195 Russell St, Hadley, MA.
