29 CFR 1926.300(b)(2): Belts, gears, shafts, pulleys, sprockets, spindles, drums, fly wheels, chains, or other reciprocating, rotating or moving parts of equipment shall be guarded if such parts are exposed to contact by employees or otherwise create a hazard. Guarding shall meet the requirements as set forth in American National Standards Institute, B15.1-1953 (R1958), Safety Code for Mechanical Power-Transmission Apparatus. a) New residential construction located on Lot #233 at 21 Silver Beech Lane, Dawsonville, GA 30534, South side of structure: On or about 07/25/2023, the employer exposed employees to severe lacerations and amputation hazards while operating a RYOBI generator, in that the recoil cover over the starter was missing which exposed operators to unguarded moving parts.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. a) New residential structure located on Lot #233 Etowah Preserve, Dawsonville, GA 30534, Front side of construction site: On or about 07/25/23, the employer exposed employees to fall hazards of approximately 15-feet, 6-inches to the lower ground level, in that no fall protection system was not provided while employees were engaged in installing siding on a pump jack scaffold system.

29 CFR 1926.403(b)(1):Examination. The employer shall ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined on the basis of the following considerations: a) New residential construction located on Lot #233 Etowah Preserve, Dawsonville, GA 30534, on the west end of the site: On or about 07/25/2023, the employer failed to inspect all electrical extension cords used to provide power to electric tools employees were using, in that an orange 50-ft. electrical cord for a circular saw and the circular saw electrical cord were damaged. An inspection of these cords was not performed prior to the shift as these cords were not repaired or removed from service.
