29 CFR 1926.451(b)(1)(i):  Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary:  a) On or about July 26, 2023, on a scaffold at the southern side of the house, employees performing brick masonry activities from a scaffold which was not fully planked.  b) On or about July 26, 2023, over the garage at the southern side of the house, employees performing brick masonry activities from a side bracket which was not fully planked.

29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:   On or about July 26, 2023, at the southern side of the house, employees were performing brick masonry activities such as but not limited to installing and cutting brick from a scaffold without a ladder or other safe means of access.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, or uprights did not bear on base plates and mud sills or other adequate firm foundation:   On or about July 26, 2023, at the southern side of the house, employees performing masonry activities such as but not limited to installing brick were on a scaffold where frames did not rest on base plates or adequate firm foundation in the following instances:  a) A leg near the front door was on a mud sill but had no base plate.  b) The leg for the other side of that frame was on a mud sill but had no base plate.  c) A leg at the corner of the garage was on a mud sill but had no base plate.   d) A leg by the garage on the roadside of the frame was not on a firm foundation and had sunk into the ground approximately 1-inch.

29 CFR 1926.451(c)(2)(ii): Unstable objects were used to support scaffolds or platform units:   On or about July 26, 2023, at the southern side of the house, employees performing masonry activities such as but not limited to installing brick were on a scaffold where frames did not rest on base plates or adequate firm foundation in the following instances:  a) A leg by the garage had a base plate that rested on stacked lumber.    b) A leg by the garage on the road side of the frame was on a mud sill that was too small for the base plate.  c) A leg by the garage side of the frame at the corner of the building was on a foundation of stacked lumber and bricks.

29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall:   On or about July 26, 2023, at the southern side of the house, employees performing masonry activities such as but not limited to installing and cutting brick were working from a scaffold that was not equipped with toeboards.
