29 CFR  1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide.  On July 24, 2023, and times prior thereto, employees engaged in cornice work on a residential home from a single pole scaffold and the work platform was not 18 inches wide exposing the employees to a fall hazard of approximately 19 feet to the ground below.

29 CFR  1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.   On July 24, 2023, and times prior thereto, employees worked from a pole scaffold without fall protection systems while installing cornice material on the exterior of a residential home. The employees were exposed to fall hazards of approximately 19 feet to the ground below.

29 CFR  1926.452(a)(4): Diagonal bracing in both directions were not installed across the entire outside face of all double- and single-pole scaffolds.   On July 24, 2023, and times prior thereto, employees working from a pole scaffold without cross bracing installed were exposed to a collapse and falling approximately 19 feet to the ground below.

29 CFR  1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe.   On July 24, 2023, and times prior thereto, the employer allowed his employees to operate skill saws on site that had small pieces of wood wedged in the guard to hold it up the guard exposing the employees to being struck -by the rotating blades.  a) The Makita skill saw #1 lower guard wedged in the up position while cutting cornice material to install on the building.  b) The Makita skill saw #2 lower guard wedged in the up position while cutting cornice material to install on the building.  c) The Makita skill saw #3 lower guard wedged in the up position while cutting cornice material to install on the building.

On July 24, 2023, and times prior thereto, employees were installing cornice material from a pole scaffold and employees, not wearing hardhats, worked from the ground passing cornice materials up to the upper level of the pole scaffold was exposed to struck-by hazards.
