Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) 116 Main Street, Bethesda United Methodist Church, Middletown, DE 19709: On or about July 26, 2023, CSHO observed employees and subcontractor's employees performing coating work on a roof of a church at heights of approximately 29 feet above the ground level without any means of fall protection.
