On or about July 22, 2023, and at times prior thereto, a job-made wooden trash box was being used with a rough terrain forklift at elevated heights without the manufacturer's approval, exposing the operator and those nearby to the hazard of being struck-by the box and materials or trash within.

On or about July 22, 2023, workers were using a stairwell with an unprotected 3rd floor landing, exposing the workers to a fall hazard greater than 6-feet to the concrete floor below.
