29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval.  On or about July 22, 2023, and at times prior thereto, a job-made wooden trash box was being used with a rough terrain forklift at elevated heights without the manufacturer's approval, exposing the operator and those nearby to the hazard of being struck-by the box and materials or trash within.

29 CFR 1926.1052(c)(12): Unprotected sides and edges of stairway landings were not provided with guardrail systems that meet the criteria contained in Subpart M of 29 CFR 1926.  On or about July 22, 2023, workers were using a stairwell with an unprotected 3rd floor landing, exposing the workers to a fall hazard greater than 6-feet to the concrete floor below.
