29 CFR 1926.20(b)(2): The employer did not initiate and maintain frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  (a)  On or about July 25, 2023,  a competent person was on site but did not perform frequent and regular inspections of the job site to ensure that hazardous conditions were identified and corrected. Employees were exposed to fall hazards from not implementing fall protection while working from the roof. This exposed employees to a 21 foot to 30 foot fall onto grass, asphalt, and concrete.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) On or about July 25, 2023, employees conducting roofing work on an 8:12 pitch roof were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 21 feet on the North and South roof sides of the roof and approximately 30 feet at the roof ridge.  J.R. Gallegos Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) , which was contained in OSHA inspection number1535821, citation number 1, item number 1 and was affirmed as a final order on 7/30/2021, with respect to a workplace located at 8013 100th Ave. Pleasant Prairie, WI.
