29 CFR 1910.110(f)(2)(ii): Liquefied petroleum gas container(s) stored inside were located near exits, stairways, or in areas normally used or intended for the safe exit of people: FMS B Vehicle Bay: On or about July 24, 2023, a (CharBroil) grill and four 15-pound propane cylinders were stored approximately 5 feet 8 inches from the main entrance leading to the FMS B vehicle bay. Employees working in the immediate vicinity and adjacent areas were potentially exposed to fire and/or explosion hazards it the propane cylinders were to accidently rupture and become a projectile or discharge due to unintentional contact with a heat source (open grill).

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial vehicle operator's performance was not conducted at least once every three years: FMS A/B: On or about July 26, 2023, refresher training was not provided every third year as required. Review of the employer's training program revealed that refresher training and operator evaluation occurred every 5 years at the facility thus potentially exposing the operator and other employees in the immediate area to struck-by hazards should the operator lose control of the fork truck.

Employees tasked with operating the drill stand directly in front of the rotating bit when machining parts, and are potentially exposed to struck-by hazards from the flying material(s).

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: On or about July 24, 2023, the opening for the work rest on the company's (Baldor) bench grinder, serial # 5415-00-517-7754 located in the FMS A Vehicle Shop at the facility, was three quarters of an inch (3/4 ") potentially exposing the operator(s) and other employees working in vicinity to caught-in hazards if the workpiece gets jammed between the abrasive wheel and work rest and/or struck-by hazards from flying projectiles or shards of the abrasive wheel if it were to break. The Nevada Army National Guard FMS - Reno was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.215(a)(4), which was contained in OSHA inspection number 1503484, citation number 01, item number 01 and was affirmed as a final order on January 6, 2021, with respect to a workplace located at 19960 Army Aviation Drive, Reno, Nevada.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: On or about July 24, 2023, the opening for the tongue guard on the Baldor bench grinder (serial # 5415-00-517-7754) located in the FMS A Vehicle Shop at the facility was one half inch (1/2 ") thus exposing the operator(s) and other employees working in vicinity to caught-in hazards if the workpiece gets jammed between the abrasive wheel and work rest and/or struck-by hazards from flying projectiles or shards of the abrasive wheel if it were to break. The Nevada Army National Guard FMS - Reno was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.215(b)(9), which was contained in OSHA inspection number 1503484, citation number 01, item number 02 and was affirmed as a final order on January 6, 2021, with respect to a workplace located at 19960 Army Aviation Drive, Reno, Nevada.
