A) 2601 Hamilton Blvd South Plainfield, NJ 07080 /Bay Door - Wall switch with live electrical parts exposed employees to electrical hazards. The switch did not have an approved enclosure to protect employees from accidental contact. Incident occurred on July 26, 2023.
