29 CFR 1926.1207(b)(5): Training was not provided by the employer when there was evidence of inadequacy in employee's knowledge of confined space procedures:  a) 350 N 8th St. Lebanon, PA 17042 - On or about July 27, 2023, employees were entering a confined space to remove fuel and sludge from a tank, and the employer did not conduct continuous monitoring or periodic monitoring of atmospheric hazards throughout the course of entry operations to ensure acceptable entry conditions are maintained.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
