29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: Guardrail systems; Safety net systems; Personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems:   On July 25, 2023, and times prior to, employees of Complete Truck and RV Repair LLC were exposed to fall hazards when working on top of recreational vehicles at 3309 New Town Boulevard in Saint Charles, Missouri. The employer did not ensure that employees utilized fall protection systems when installing items such as, but not limited to, insulation, air conditioner units, communications and electrical.  This work is performed approximately 10-12 feet above the lower level.

The employer did not ensure that employees received medical evaluations before giving and instructing employees to wear 3M half mask respirator with organic vapor cartridges when painting and performing body work.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:   On July 25, 2023, and times prior to, employees of Complete Truck and RV Repair LLC utilized a fork attachment on a Hyundai 30L-7A powered industrial truck at 3309 New Town Boulevard in Saint Charles, Missouri. The employer did not ensure modification and additions affecting the capacity of the powered industrial truck received manufacturers approval before use.  Employees utilized the fork attachment affixed with a ball to move recreation trailers around the property.
