29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. St. Mary's Iron Works, Inc. dba Iron Works, Inc. - Fort Loramie, Ohio: On or about July 19, 2023, the employer did not ensure that employees were protected from rotating part hazards while operating the Chevalier milling machine (model FM-32HP; s/n HP872004). Employees who operated the machine were exposed to amputation injury from being caught-by the unguarded chuck and cutter tooling. b. St. Mary's Iron Works, Inc. dba Iron Works, Inc. - Fort Loramie, Ohio: On or about July 27, 2023, the employer did not ensure that employees were protected from rotating part hazards while operating the Bridgeport milling machine (model 6-X005). Employees who operated the drill press were exposed to caught-by hazards from the unguarded chuck the rotating bit when wearing gloves during operation. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

St. Mary's Iron Works, Inc. dba Iron Works, Inc. - Fort Loramie, Ohio: On or about July 26, 2023, the employer failed to report an amputation injury to an employee. On July 19, 2023, an employee sustained a partial finger amputation injury; the employer did not report the amputation within twenty-four hours. No additional abatement information is needed for this item.
