29 CFR 1910.28(b)(3)(i): The employer did not ensure that each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.28(b)(3)(i)(A)-(D). On or about July 26, 2023, employees were performing cleaning work activities on top of a roof with skylights. The skylights were not covered or protected with guardrails , nor did the employees have a personal fall arrest system or travel restraint system. This exposed the employees to fall hazards greater than four (4) feet to the ground below.

29 CFR 1910.28(b)(13)(i): The employer did not ensure that when work is performed less than 6 feet (1.6 m) from the roof edge, each employee is protected from falling by a guardrail system, safety net system, travel restraint system, or personal fall arrest system. On or about July 10, 2023, employees were performing cleaning work activities from a roof without the use of fall protection. This exposed employees to a fall greater than four (4) feet to the ground below.

29 CFR 1910.30(a)(1): The employer did not provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart before any employee is exposed to a fall hazard. And/or the employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017. On or about July 17, 2023 and at times prior thereto, employees were working at heights and were not trained to recognize the hazards associated with falls in general industry. This exposed the employees to a fall hazard.
