29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On or about July 26, 2023 and at times prior thereto, employees were working from a roof of a residential building and were not in use of fall protection systems. This exposed the employees to a fall hazard greater than six (6) feet to the ground below.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. On or about July 23, 2023, the employees were in use of a ladder that was not long enough for its siderails to extend at least three feet above the landing. This is a fall hazard for the employees.

29 CFR 1926.1053(b)(6): Ladders were used on surfaces which were not stable and level. On or about July 26, 2023, employees were in use of a ladder that was placed on top of a stack of shingles to reach to the top of the roof. The employees used this ladder to access the roof and this exposed the employees to a fall hazard.
