29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i): a) Jobsite, Warehouse, 115 Downieville Rd, Mars, PA 16046: On or about June 26, 2023, employees who operate the palletizer are exposed to fall hazards while removing product and boxes from the palletizer table. The edge of the palletizer table opposite the operator's station was approximately 7 feet 2 inches above the shop floor. The edge of the palletizer table nearest the operator's station was approximately 4 feet 2.5 inches above the walkway.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: a) Jobsite, Warehouse, 115 Downieville Rd, Mars, PA 16046: On or about June 26, 2023, employees who operate the palletizer were not trained as Authorized employees under the employer's energy control program. Employees who operate the palletizer are periodically required to climb onto the palletizer table to remove product when the machine gets out of sequence or to remove products stuck on the table. The palletizer is not locked and tagged out when employees climb onto the machine.
