On or about July 28, 2023, the employer did not provide the required location (column E) and/or injury and illness description (column F) on the OSHA Form 300 or equivalent for the calendar year 2023. (a) On or about June 5, 2023, while working in Mesa, AZ, an employee sustained a fractured ankle, case no. 6. (b) On or about June 12, 2023, while working in O'Fallon, MO, an employee sustained a bicep tear, case no. 7. (c) On or about July 27, 2023, while working in Eau Claire, WI, an employee loss consciousness, case no. 8.
