29 CFR 1910.147(c)(4)(i):Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about July 13, 2023, at 700 16th Street SE, Massillon, OH 44646, the employer failed to ensure employees unjamming the liner perform lockout/tagout prior to performing maintenance. An employee was attempting to remove a jammed lid from the liner and reached into the machine without performing lockout/tagout.
