29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems.  On or about July 31, 2023, and at times prior thereto, employees were accessing multiple stages of the paint line at height without the use of fall protection, exposing the employees to the hazard of falling over 4-feet to the concrete slab below.

29 CFR 1910.147(c)(7):  The employer did not provided training to ensure that the purpose and function of energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are required by employees.  On or about July 31, 2023, and at times prior thereto, employees were cleaning machines within the facility without training on energy control procedures, exposing the employees to the hazard of being caught-in the machines from the release of stored energy or unexpected startup.

On or about July 31, 2023, and at times prior thereto, employees were operating Dayton bench grinder(s) without the use of an adjustable tongue guard, exposing the employees to the hazard of being struck-by flying debris and projectiles.

29 CFR 1910.252(b)(2)(i)(C): All operators and attendants of resistance welding or resistance brazing equipment did not use transparent face shields or goggles, depending on the particular job, to protect their faces or eyes, as required.  On or about July 31, 2023, and at times prior thereto, employees were operating a resistance welder machine without the proper face protection, exposing the employees to the hazard of face, tongue and mouth burns.
