29 CFR  1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  WORKSHOP:  On/or about March 15, 2023, an employee was removing paper from the Jennerjahn machine while in operation at the tuck bar and roller, resulted in two finger crushes requiring medical treatment.  The employer had not established step-by-step hazardous energy control lockout/tagout procedures in the machine specific for the employees during servicing and maintenance of equipment such as (but not limited to) the Jennerjahn machines.

29 CFR  1904.29(b)(3):Each recordable injury or illness was not recorded on the OSHA 300 Log and 301 Incident Report within seven calendar days of receiving information that a recordable injury or illness has occurred:  ESTABLISHMENT:  On/or about August 1, 2023, the employer had not recorded the employee requiring medical treatment within seven calendar days on the OSHA 300 log of work-related injury or illness and the OSHA 301 incident report.    a)  On or about March 15, 2023, an employee sustained a crushing injury to two fingers which resulted in 71 days away from work.  The injury would be appropriately captured with a check in Column H (Days Away From Work), 71 days away in Column K, and a check in Column M1.  ABATED DURING THE INSPECTION.
