Heirloom, Cumming, GA 30040: On or about 07/25/2023, the employer exposed employees to severe lacerations and amputation hazards while operating a HERC trailer cement mixer, in that the recoil cover over the starter was missing which exposed operators to unguarded moving parts. b) New residential construction on Lot #24 at 5150 N. Heirloom, Cumming, GA 30040, North side of structure: On or about 07/25/2023, the employer exposed employees to severe lacerations and amputation hazards while operating a Craftsman Generator, in that recoil cover was missing which exposed operators to unguarded moving parts.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii). a) New residential construction on Lot #24 at 5150 N Heirloom, Cumming, GA, 30041: On or about 07/25/2023, the employer exposed employees to fall hazards of approximately 15-feet to the lower ground level, in that the scaffold work platforms were not fully decked to prevent falls to the lower ground level as employees were engaged in masonry activities.
