29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use: a) 519 Vosseller Ave, Bound Brook, NJ 08805- Employee(s) stood and worked atop a job-made wooden work platform (scaffold), which was elevated and supported on the extended fork carriage of a JLG telehandler boom, installing fascia boards at the roof line of a 3-story new residential property under construction. The employer's job-made (scaffold) platform was not designed for the manufacturer for that purpose; nor was entire platform attached to the forks, as per 1926.451(c)(2)(v), exposing employees to a fall hazard. On or about 8/2/2023, the movement and weight of employee(s) working together on the supported scaffold platform caused a load imbalance that tilted/shifted it, resulting in the employee(s) falling 35 ft to the ground below and incurring serious injuries. NOTE: Written documentation of abatement is required for this citation item, per 1903.19(d)(1).

29 CFR 1926.454(a):The employer did not have each employee who performs work while on a scaffold(s) trained by a person qualified in the subject matter to recognize those hazards associated with the type of scaffold(s) being used and to understand the procedures to control or minimize these hazards: a) 519 Vosseller Ave, Bound Brook, NJ 08805- Employee(s) stood and worked atop a job-made wooden work platform (scaffold), which was elevated and supported on the elevated fork carriage of a JLG telehandler boom, installing fascia boards at the roof line of a 3-story new residential property under construction. The employee(s) working atop this supported scaffold were not provided training on required measures for preventing falls & electrical hazards associated with such scaffold work. On or about 8/2/2023, the movement and weight of employee(s) working together on the supported scaffold platform caused a load imbalance that tilted/shifted it, resulting in the employee(s) falling 35 ft to the ground below and incurring serious injuries. 29 CFR 1926.451(f)(6):Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6): a) 519 Vosseller Ave, Bound Brook, NJ 08805- Employee(s) stood and worked atop a job-made wooden work platform (scaffold), which was elevated and supported on the extended fork carriage of a JLG telehandler boom, installing fascia boards at the roof line of a 3-story new residential property under construction. The employer did not ensure the the wooden (scaffold) platform was constructed and used such that all person(s) and equipment would maintain a minimum clearance of 10 feet away from energized power lines, exposing employees to electrical contact hazard(s). On or about 8/2/2023, employee(s) suffered non-fatal injuries when the telehandler boom was moved and the platform struck against the cross arm of the powerlines and fell approximately 35 ft to the ground.
