29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolding was not fully planked or decked:  a.) On or about August 03, 2023, at 154-10 Powells Cove Boulevard, BEECHHURST, NY 11357.   Employees  applying stucco on the 2nd floor exterior facade from a tubular welded frame scaffold was missing three planked with a gap of roughly 18 inches. The working level was approximately 30 feet above the ground level.   ABATEMENT CERTIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.

29 CFR 1926.451(e)(1): Where scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers, stairway-type ladders, ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used for access:  a.) On or about August 3, 2023, at 154-10 Powells Cove Boulevard, BEECHHURST, NY 11357.  Employees  applying stucco on the 2nd floor exterior fa�ade on a tubular welded frame scaffold did not have a means of access or egress to the scaffold.   The working level was approximately 30 feet from the ground level.  ABATEMENT CERTIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  a. On or about August 3,  2023, at 154-10 Powells Cove Boulevard, BEECHHURST, NY 11357.  Employees working on residential rooftop approximately 16 feet above the lower level  installing stucco were not protected from falling to the lower level.   ABATEMENT CERTIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.

On or about August 3, 202 at 154-10 Powells Cove Boulevard, BEECHHURST, NY 11357.  Employees  applying stucco on a 2nd floor exterior fa�ade from a tubular welded frame scaffold were not protected from a falls hazard by the use of fall arrest system or guardrail system.  The working level was approximately 30 feet from the lower level.  ABATEMENT CERTIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.
