29 CFR 1904.41(a)(2): The establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and the establishment was classified in an industry listed in appendix A to subpart E of this part, and the employer did not electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee by the specified date: a) This violation was discovered on or about August 3, 2023, at the workplace, where the employer failed to electronically submit information from their calendar year 2022 OSHA Form 300A or equivalent by March 2, 2023. This establishment employed 72 employees and was classified in 326199 (North American Industrial Classification System code for All other Plastic Product Manufacturing) during calendar year 2022. 3D Plastics, LLC dba Quantum Plastics was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1904.41(a)(2)), which was contained in OSHA inspection number 1398994, citation number 1, item number 1 and was affirmed as a final order on July 12, 2019, with respect to a workplace located at 1095 East Commerce Avenue, Gladewater, Texas 75647.
