29 CFR  1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition:  On or about August 3, 2023, the employer failed to keep the walking working surfaces near the flute and former machines cleaned of oil and other residue which is causing slippery surfaces.  Employees are exposed to slip and fall hazards when walking or working near the flute and former machines.

29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces are inspected, regularly and as necessary, and maintained in a safe condition:  On or about August 3, 2023, the employer failed to regularly inspect the walking working surfaces to ensure they are in clean and safe condition.  The walking working surfaces near the flute and former machines have a build up of oil, lubricants and other machinery fluids thus making the surface areas slippery.  Employees are exposed to slip and fall hazards when walking or working near the flute and former machines.

29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   On or about August 3, 2023, the employer failed to ensure that employees who were tasked with making adjustments to the rollers on the Flute Machines utilized lockout procedures prior to servicing the equipment in that they powered off the equipment without locking it out. Employee are exposed to stuck by and/or caught in between hazards of the moving chain and sprockets.

29 CFR  1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed:  On or about August 3, 2023, the employer failed to provide effective guarding on the sprocket and chains of the power transmission on the flute Machine, including but not limited to Flute #2.  Employees are exposed when they are making adjustments to the rollers via a hand wrench several times per shift.
