29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee was using eye or face protection when exposed to eye or face hazards from flying particles.   a. On the roof of a temporary residential worksite, located at 322 West 61st Street, Jacksonville, FL 32208: On or about August 1, 2023, the employer exposed an employee to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by employees, while using a nail gun to install roof shingles.  Pinnacle Roofing Group, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1658038, citation number 01, item number 001 and was affirmed as a final order on April 19, 2023, with respect to a workplace located at 709 Butler Blvd, Jacksonville, FL 32218.

29 CFR  1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   a.  On a 5:12 pitched roof of a temporary residential worksite, located at 322 West 61st Street, Jacksonville, FL 32208:  On or about August 1, 2023, the employer exposed two employees to fall hazards, in that, fall protection was not utilized by the employees while performing roofing activities.  Pinnacle Roofing Group, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CR 1926.501(b)(13), which was contained in OSHA inspection number 1658038, citation number 01, item number 002 and was affirmed as a final order on April 19, 2023, with respect to a workplace located at 709 Butler Blvd, Jacksonville, FL 32218.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:  a. On a temporary residential job site located at 322 West 61st Street, Jacksonville, FL 32208: On or about  August 1, 2023, the employer exposed employees to fall hazards 9-foot, in that the ladder side rails of a 20-foot multi-purpose extension ladder were not extended 3 feet above the roof eve.  Pinnacle Roofing Group, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1658038, citation number 01, item number 003 and was affirmed as a final order on April 19, 2023, with respect to a workplace located at 709 Butler Blvd, Daytona Beach, FL 32118.
