29 CFR 1910.147(c)(4)(ii): Energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items a-d of this section: On or about August 4, 2023, employees engaged in servicing and maintenance activities at the Bimbo Bakeries USA, Inc. located at 1310 Fort Crook Road, Bellevue, Nebraska, were exposed to amputation and caught-in hazards that the employer failed to identify the specific steps employees must take to shut down equipment/machinery prior to energy control and isolation, and the specific steps required to validate or verify the effectiveness of the lockout/tagout device(s) (such as how to operate equipment/machinery to verify de-energization). Instances included but were not limited to: i. Bun Divider 1 & 2 ii. Bun Slicer iii. Basket Rotary Denester

29 CFR 1910.147(c)(6)(ii): The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection: On or about August 4, 2023, employees engaged in servicing and maintenance activities at the Bimbo Bakeries USA, Inc. located at 1310 Fort Crook Road, Bellevue, Nebraska, were exposed to amputation and laceration hazards in that the employer failed to certify lockout/tagout periodic inspections were performed for several procedures to include but not limited to the bun slicer.
