Location: Worksite On July 28, 2023, an employee's foot was injured when it was caught between the pallet jack and metal bar of the storage rack while he operated a pallet jack. Employees were exposed to caught-in or between and other hazards while operating various powered industrial trucks and the employer had not ensured that all powered industrial truck operator training topics were taught to the employees.
