29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about July 28, 2023, the employer did not provide a training program for employees exposed to a fall of 8.7 feet above the ground while they were installing shingles on a residential roof located at 875 Hanover CT Akron, OH 44313, to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about July 28, 2023, employees were exposed to a fall hazard of 8.7 feet above the ground while installing shingles on the roof of a residential home located at 875 Hanover CT Akron, OH 44313, and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure.   Benedicto Licea Segura DBA Segura Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1412464, citation number 1, item number 3 and was affirmed as a final order on 8-20-2019, with respect to a workplace located at 5603, 5605, 5607, 5609 Rainbow Falls Street, DUBLIN, OH 43016.   Benedicto Licea Segura DBA Benie Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1338112, citation number 1, item number 2 and was affirmed as a final order on 4-18-2019, with respect to a workplace located at1120 Eagle Feather Circle Dayton, OH 45444.
