On or before August 2, 2023, employees were exposed to a fall of approximately 22 feet while performing air-conditioning maintenance on the roof of Grace Church located at 1055 Canton Road, Akron, Ohio 44312. Maintenance employees performed the work 1-3 times a month for the last 23 years and the employer did not ensure a safe access point was provided.

On or before August 2, 2023, employees were exposed to a fall of approximately 8 feet while performing air-conditioning maintenance on the roof of Grace Church located at 1055 Canton Road, Akron, Ohio 44312. Maintenance employees used an A-frame ladder to gain access to the roof and the employer did not ensure the ladder was used for the purposes it was designed.
