At 2023 Westport Center DR, Saint Louis, MO 63146, On August 8, 2023 and at times prior thereto, employees were exposed to struck by and caught between hazards associated with the operation of powered industrial vehicles due to the employer not ensuring that each operator was competent to operate a powered industrial truck safely by the successful completion of training and evaluation.
