Written procedures were not developed for equipment such as but not limited to the Mecanor Mecaring Machine (Asset # 000516) and Internally built "Bender" machine(s) (Asset # 000475) at the employer's 2929 W Venture Dr. Janesville, WI 53546 packaging cell department. a) Hazardous energy control procedures were not developed for the Mecanor Mecaring Gasket Installation machine for maintenance and servicing activities. b) Hazardous energy control procedures were not developed for the Bender Machine(s) when employees are engaged in maintenance and servicing activities such as adjustments.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about August 8, 2023, one or more methods of machine guarding was not provided to protect Mecanor Gasket Insertion Machine (asset # 000516) operators from hazards created by the point of operation. Employees are exposed to an amputation hazard when inserting gaskets into PVC tubes approximately 3-4in from the point of operation.
