29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. (a) Worksite: On or about 08/09/2023, employees were exposed to overhead, struck-by hazards while utilizing a mobile crane to hoist materials and equipment from the ground level to a commercial roof, approximately 26 feet in height, without protective helmets.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) Worksite: On or about 8/09/2023, employees were exposed to fall hazards while unloading material from a crane onto a commercial roof, at a height of approximately 26 feet above the lower level, without any means of fall protection.
