On a roof, at a temporary residential construction site, located at 6467 Heidi Road, in Jacksonville FL: On or about August 9, 2023, Taurus Recovery Service, LLC., failed to ensure that its subcontractor Brayden Roofing Service, LLC., protected its employees from fall hazards, in that fall protection was not utilized by subcontracting employees while performing roofing operations.
