29 CFR 1926.20(b)(1):The employer did not initiate and maintain accident prevention programs as necessary to comply with this part: a) 405 Middletown Warwick Road, Middletown, DE 19709 (Parking Lot): On or about August 9, 2023, CSHO observed employees working on site to conduct sign installation operations exposed to multiple hazards including but not limited to falls, burns, and struck by hazards which had not been identified due to a lack of an accident prevention program onsite.

CSHO observed employee leaning over parapet wall that measured 3 feet.
