29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (a) At the laundry room: On or about August 9, 2023- the employer exposed employees to chemical hazards, in that employees worked with chemicals including, but not limited to, Oxyclean Laundry Stain Remover, Tide Power Pods, and Shout Stain Remover had not been trained. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1904.41(a)(2): Annual electronic submission of OSHA Form 300A Summary of Work-Related Injuries and Illnesses by establishments with 20 or more employees but fewer than 250 employees in designated industries. If your establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and your establishment is classified in an industry listed in appendix A to subpart E of this part, then you must electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form: (a) On or about August 9, 2023 - Holiday Inn Express & Suites Pell City, an IHG Hotel, 240 Vaughan Lane, Pell City, AL 35125, the employer failed to electronically submit information from the OSHA Form 300A or equivalent for calendar year 2022 by March 02, 2023.
