29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about August 8, 2023,  in the back of the production area, where an employee was exposed to caught-by hazards while operating a food conveyor while wrapping boudin egg rolls.

29 CFR  1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent.    On or about August 8, 2023, at Frenchy's Sausage Company Inc located 4220 Pinemont,  Houston, Texas the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year, 2023.  The establishment employed 60 employees and is classified 331119 during calendar year 2023.  a)  On 6/21/2023,  an employee crushed and fractured his right and middle finger when a delivery truck door closed on his hand.    b)  On 7/27/2023,  an employee crushed right arm and hand when caught in a conveyor belt while removing a struck object.

29 CFR  1904.41(a)(2) Annual electronic submission of OSHA Form 300A Summary of Work-Related Injuries and Illnesses by establishments with 20 or more employees but fewer than 250 employees in designated industries. If your establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and your establishment is classified in an industry listed in appendix A to subpart E of this part, then you must electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form.   On or about August 8, 2023, at the  Frenchy's Sausage Company Inc., located at  4220 Pinemont,  Houston, Texas; the employer failed to electronically submit information from their  during calendar year 2022,  OSHA Form 300A or equivalent by 03/01/2023.
