29 CFR  1926.501(b)(4)(i):Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  a) On or about August 10, 2023, located on an 8/12 commercial metal roof, the employer failed to ensure that employees were utilizing an effective fall protection system while performing roofing work around an unguarded roof hole. As a result, employees were exposed to a fall hazard of approximately 47 feet above ground level.
