Location: 44 Oakland Ave, Methuen, MA; O or about 08/10/2023, the worker and foreman were witnessed utilizing a fall protection system anchorage which had only been secured to the roof with two fasteners per side (four total), and was not installed per the manufacturer instructions to support at least 5,000lbs per employee attached.
