29 CFR 1926.451(g)(1):Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level. On or about Aug 10, 2023, on the east side of the project site, employees were on scaffolding with no guardrail system installed while performing masonry work on the exterior of the building at heights greater than ten (10) feet.

29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects. For falling objects too large, heavy, or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall. On or about August 10, 2023, on the north side of the site, employees were performing masonry on a scaffold without toeboards to protect them from overhead falling objects.
