On or about 02/15/2023, at job site 17900 SW 100th Street Miami, FL 33196, the employer did not maintain an accident prevention program to ensure employees of its subcontractors were protected from fall and struck by hazards when performing welding operations from a basket that was not secured to the forks of all terrain forklift.
