29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access: On or about August 10, 2023, employees doing roof work on a two stories single-family house were exposed to fall hazards by using extension ladders that did not extend 3-feet above the landing of the roof. (a) East end: Werner 28-feet extension ladder, model no. D1328-2, front of the house. Employees were exposed to 20-feet fall hazard. (b) North end: Werner 16-feet extension ladders at side of the house. No model number and serial number could be recognized. Employees were exposed to 10-feet fall hazard. (c) North end: Werner 20-feet extension ladders at side of the house. No model number and serial number could be recognized. Employees were exposed o 10-feet fall hazard. (d) South end: Werner 16-feet extension ladder at the side of the house. Employees were exposed to 10-feet fall hazard. (e) West end: Werner 16-feet extension ladder at back of the house. Employees were exposed to 10-feet fall hazard.
