29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles:  On or about 08-11-2023, at a residential construction jobsite located at 25 Green Meadows Drive, Sikeston, MO., employees were exposed to a struck by hazard. Employees were observed actively installing new roofing material while operating pneumatic nail guns without any form of eye or face protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about 08-11-2023, at the residential construction jobsite located at 25 Green Meadows Drive, Sikeston, MO., employees were exposed to a fall hazard. Employees were observed actively installing new roofing material, while actively working approximately 15 feet above ground.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper-level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed:  On or about 08-11-2023, at the residential construction jobsite located at 25 Green Meadows Drive, Sikeston, MO., employees were exposed to a fall hazard. Employees were observed using the ladder improperly while climbing on the aluminum ladder rungs. The crew was using an Aluminum Extension Ladder improperly as the ladder side rails did not extend at least three feet (.9 m) above the upper landing surface.
