29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: ' On or about July 28, 2023 at 600 East Clark Street, East Palestine, OH 44113, the employer failed to ensure employees performing trimming operations on the grey piston extruder was protected against being exposed to a moving piston/ram in that employees reach inside the chamber area to trim flashing from the product.

29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about July 28, 2023 at 600 East Clark Street, East Palestine, OH 44113, the employer failed to ensure employees operating a piston extruder was protected against contact with the cut flashing piston in that there is no machine guarding or other means to protect employees in place. An employee received an amputation when their finger came into contact with the ram piston.
